Service Provision

Transform Forth Valley offer a wide range of services to support individuals and families who are impacted by substance use and/or societal, financial or health inequalities. We respond to identified need in Forth Valley and in partnership we support individuals and families to tackle Inequalities that diminish life chances. We consider the whole person, building a firm foundation from which an individual or family can achieve positive and lasting change.

We engage with people at the earliest opportunity to prevent issues from worsening. We provide direct intervention and support to meet personal objectives. We work in partnership and provide coordination to ensure all needs are being met.

Our aims and objectives are Identify and support the most vulnerable and hard to reach individuals in Forth Valley, develop the awareness of the impact of substance use on young people, children and families, increase self-confidence, self-management and self-determination,  improve access to the right support services, work in partnership to achieve better life chances and improved outcomes.

Social Inclusion Project (SIP)

The Social Inclusion Project (SIP) works with individuals over 16 across Forth Valley to enable and empower them to access support and engage with community services. Those accessing the service have experience of problematic substance use, are currently offending or involved with anti-social behaviour and are not on a Justice order.

We work with individuals with a wide range of complex issues and often unstable lifestyles, who are either not involved with, or find it difficult to stay engaged with mainstream services. We carry out a full assessment of need and develop a personalised and unique plan of support. This may include assistance to access local services, addressing housing needs, accessing benefits, treatment for drug or alcohol issues, referrals for mental health support, or engagement with the community justice teams. As independence and stability increase, involvement with SIP will reduce and ultimately cease.

 This is a professional only referral service for individuals not on a Justice Order.

Transform Forth Valley Tackling Inequalities, Poverty and Improving Outcomes Project (Falkirk)

TIIOP is part of the Social Inclusion Project, working in partnership to support individuals who are currently involved with Falkirk Council Justice Service.

The TIIO Project works with individuals aged 16+ years who reside within the Falkirk Council Area who are: on a current statutory order with Falkirk Justice Service such as a Community Payback Order or a Structured Deferred Sentence, subject to post-release supervision licences such as parole or a non-parole licence or, referred via the CJS Diversion from Prosecution scheme.

Engagement with TIIOP is voluntary. TIIOP provides intensive support, based on a whole person response. Wide ranging assistance is provided with the aim of addressing not only the effect, but also the cause of the challenges being experienced.

TIIOP works closely with clients to identify the changes important to them and establish a tailored support plan. We collaborate with a wide range of agencies to help an individual to address these needs, and ensure they are delivered in a coordinated and seamless way. We seek to break cycles of offending, remove barriers to care and tackle inequalities whether they be societal, economic or in relation to health.

Housing First Project (Falkirk)

The Housing First team provides secure and settled housing as the first response for individuals with multiple and complex individual needs. Eligibility is simply that the individual is willing and committed to their own tenancy agreement, and the home provided for as long as needed. Housing First is offered to individuals who have been homeless before but who struggle without additional support.

The Housing First team provides housing stability, and long-term individualised support, allowing an individual to build a positive and sustainable future. Individuals accessing the service will design their own support, at their own pace, and how they want the service to help.

We work with individuals to identify goal that are important to them; and engage with a wide range of health, social care, housing, and support services to ensure the right assistance is in place. There is no fixed end date; we acknowledge that recovery takes time and varies in relation to individual need and experiences.

The Self Neglect and Hoarding Service (Falkirk)

The Self-Neglect & Hoarding Service is based in Falkirk and works with Falkirk Council Adult Support & Protection Service. The service responds to concerns of hoarding within peoples living spaces. The service aims to address presenting issues and reduce the likelihood of difficulties or harm from escalating in the future.

The Self-Neglect & Hoarding Service offers support to individuals and families who are struggling with hoarding issues within their own living space. We work with adults aged 18 and over, who are willing to engage to receive support that is related to hoarding and wider issues.

We work with adults experiencing hoarding in their living spaces to explore how we can help make their living space safer. We offer support on housing issues, finances, substance use and / or health care and can make referrals to other services as required. We also work together with other Professionals to ensure the support offered is comprehensive and holistic.

We work in partnership with Falkirk Council Adult Support & Protection Service and partners to provide support when problems have been identified. We work with individuals on a voluntary basis to reduce risk and focus on building and supporting personal strengths, abilities, and resources. We have no time limit to this support and will go along at the individual’s pace. Together with other services we offer a collaborative approach to both practical and emotional support to individuals and whole families.

To refer contact us for a Practitioners Hoarding Assessment Form which will be used to make a referral for a supportive intervention; Complete this form then then submit to Falkirk Adult Social Services for allocation. Assertive Outreach Service

Assertive Outreach Plus works across the Forth Valley area to support individuals over 16, their families, and the local community experiencing challenges whether that be financial, societal or in relation to their health and wellbeing.

Assertive Outreach Plus operates a Mobile Outreach Vehicle allowing us to work within any Forth Valley community, and with a focus on prevention and early intervention.

We work with individuals with a wide range of complex issues and often unstable lifestyles, who are either not involved with, or find it difficult to stay engaged with mainstream services. Visiting individuals in their own homes, in community drop ins with partners, street foot patrol or by using the Mobile Outreach Vehicle.

Using a holistic approach working in partnership with the individual, AO+ work closely with other services to put in the place the support that is wanted or needed by an individual to make the greatest impact. We work flexibly to support individuals at different stages of their life and will discuss how often we meet and where. We understand that needs change and we can respond to this as and when necessary. The assistance offered may range from accessing local services, addressing housing needs, accessing benefits, support to access treatment for drug or alcohol issues, referrals for mental health support, or engagement with the community justice teams.

Time for Us

Time 4 Us works across Forth Valley providing support to children and families who have been impacted by substance use. The service aims to increase parenting capacity and enhance children’s self-esteem, confidence and resilience whilst supporting families to make positive choices. We aim to assist families to stay together, where it is in the child’s best interest to do so.

Time 4 Us offers support and focused interventions to children and families affected by a significant family member or carer’s drug and or alcohol use. We work with adults who have care of, or contact with, their children aged 0-16 years and who are stable in their recovery and engaging with substance use services. We also support children and young people aged 5-16 years who have been impacted by the substance use of a family member.

We work in partnership with children, families and professionals ensuring the best outcomes are achieved together.

We conduct a full individual or family assessment and develop a comprehensive package of care and support. We use play and focused interventions to support children to explore their experiences, their world and their emotions. We also work closely with parents and carers to achieve positive parenting skills.

Time 4 Us Early Intervention

The Early Intervention Service works across the Forth Valley area responding to low level concerns which have come to the attention of Police Scotland about children, young people, and families. These concerns have been referred to Time 4 Us for help and support. The service aims to provide support to reduce the likelihood of difficulties or harm from escalating in the future.

We are an Early Intervention service supporting children, young people, and families across the Forth Valley area. We provide support where substance use, mental health issues and other difficulties have been identified. We understand that from time to time family life can have its complications and at these times we offer early help and extra support to solve the challenges or problems being experienced.

We offer support to children, young people, and families, as soon as we become aware of their concern. We know early help and support can give individuals the tools to solve the challenges or problems they are experiencing and prevent them from escalating. We work in partnership with families to enable a rights-respecting, strengths-based, inclusive approach with the full participation of children, young people, and families in decision-making.

Our Whole Family Approach helps provide adults and children with the tools they need to create plans and set and achieve goals together. We work in a non-judgemental way, with understanding and compassion, to support families at their time of need. We work in partnership with Police Scotland and other professionals to identify and provide effective early support when problems first emerge. We work with individuals on a voluntary basis to reduce risk and focus on building and supporting personal strengths, abilities, and resources.
